This policy explains what information we collect on this website, why we collect it, who we share it with, and the choices you have. "We," "us," and "our" mean Jamel Lewis, an independent licensed life insurance agent in Illinois (NPN 20464462).
What we collect
What you give us. When you fill out our "Request a call" form, we collect:
- Your name, phone number, and email address
- The type of coverage you're interested in
- Any message you choose to write
- Whether you checked the box agreeing to calls and texts, plus the date, time, and wording you agreed to
The form asks you not to share medical details, Social Security numbers, or account numbers. We don't want those through the website.
What we learn during a consultation or application. If you choose to apply for coverage, we collect what the insurance company needs, by phone or through the insurance company's own application. This can include your date of birth, health history, medications, beneficiary details, and payment information for the insurance company.
Email signup. This site does not currently have an email newsletter signup. If we add one, this policy will be updated.
Analytics, pixels, and cookies. We do not currently use analytics tools, advertising pixels, or tracking cookies on this site. If we add any, we will update this policy first. Our website host and our font provider (Google Fonts) may automatically receive basic technical information such as your IP address and browser type when pages load.
Why we collect it and how we use it
- To call, text, or email you back about the coverage you asked about
- To compare coverage options and prepare quotes for you
- To help you apply for coverage and service your policy after it's issued
- To keep records that insurance rules and our agreements with insurance companies require
- To send texts or automated calls only if you agreed to them
- To understand how the website is used and improve it, if analytics tools are used
Who we share it with
We share your information only as needed to help you:
- Insurance companies you apply with, so they can review your application, check records such as prescription history with your permission, and issue your policy
- Our insurance marketing organization and agency, Lasting Mark and Modern Financial, which process applications and support our business
- Our CRM, GoHighLevel, which stores contact details and notes and sends our emails, calls, and text messages
- Government agencies or others when the law requires it
Payments. We do not take payments on this website and do not use a payment processor here. Premiums are paid directly to the insurance company that issues your policy.
Your phone number and text consent are never sold. We do not sell or share your phone number or your text message consent with any third party for their marketing. Service providers that help us send messages may only use them to provide that service to us.
We do not sell your personal information.
Calls and text messages
If you check the consent box, you agree to calls and texts about life insurance, including ones sent using automated technology. Agreeing is not required to buy anything. Msg & data rates may apply. Msg frequency varies. Reply STOP to any text to opt out. Reply HELP for help. You can also call (312) 993-9845 or email coverage@jamellewissr.com to stop calls or texts.
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging opt-in data and consent are excluded from all sharing described in this policy and will not be shared with any third parties.
How long we keep it
We keep information as long as we need it to help you, and as long as insurance rules and our agreements with insurance companies require. Some of those agreements require records to be kept for at least five years.
How we protect it
We use password-protected accounts and limit access to people who need it. No website or system is perfectly secure, so please don't send sensitive information through the website form.
Your choices: access, correction, and deletion
You can ask us to:
- Tell you what personal information we have about you
- Correct information that's wrong
- Delete your information
- Stop calls, texts, or emails
To make a request, email coverage@jamellewissr.com or call (312) 993-9845. We may need to confirm who you are first. We'll confirm we received your request within 2 business days and complete it within 30 days. If we can't delete something because the law or an insurance company requires us to keep it, we'll tell you.
Children
This site is not meant for anyone under 18, and we do not knowingly collect information from children through it.
Changes to this policy
If we change this policy, we'll post the new version here with a new effective date.
